Looking For A Filler Alternative? Why You Should Try Biostimulators

Looking for an alternative to filler that gives you really natural results and a skin quality boost? We might have just the thing! Biostimulators are one of this year’s buzziest injectable treatments and in the right hands they can help you smooth wrinkles and treat signs of ageing across your full face.

We spoke to two leading aesthetic doctors, Dr Ashwin Soni and Professor Bob Khanna to discover everything you need to know about the trending group of injectables.

Firstly, what are biostimulators?

There’s a hint in their name. “Biostimulators are injectable products that stimulate your own body’s cells to increase things that we lose with time and age,” explains the plastic surgeon and founder of The Soni Clinic, Dr Ashwin Soni.

That can have a pretty effective anti-ageing effect on your skin. “Biostimulators are used to boost collagen production,” says the facial aesthetics expert Professor Bob Khanna. And, this can “lead to better quality skin in terms of texture, tone, glow, fewer fine lines and wrinkles.”

What types of biostimulator are best?

“There are a variety of biostimulators,” says Dr Soni. You can find out about each type in our comprehensive biostimulator guide.
Dr Soni works with the injectable Sculptra by Galderma, which has been shown to boost type 1 collagen by 67% in 12 weeks, while also providing a natural lift to facial tissues. Professor Khanna favours the animal-derived versions of polynucleotides (derived from salmon sperm) and exosomes (derived from antlers) – which you can also learn more about in our dedicated polynucleotides and exosomes pages.

How are they different from fillers?

In simple terms: filler tackles signs of ageing by restoring volume. Biostimulators work by improving skin quality.

“Filler is a true volumiser, often made up of cross-linked hyaluronic acid, which can restore structure and support to our facial tissues,” explains Dr Soni, who favours Restylane by Galderma for this purpose.
“Biostimulators are similar in that they too help restore some structure, but they are not true volumisers,” Dr Soni continues. “Instead, they boost collagen and elastin in the skin and tissues. They will also improve pore size, skin tone, and skin quality, which filler cannot do.”

Why are biostimulators so popular right now?

One reason is that most of us are done with ‘looking done’. So, there’s been a big shift towards boosting what we naturally have, rather than necessarily plumping up our faces.

“When you look at current trends in our industry, you will see a huge rise in the search of collagen and collagen-boosting treatments,” affirms Dr Soni. “Patients want more subtle and natural results. The beauty of biostimulators is that you can obtain a really beautiful result, which will also help with the facial ageing process. These products will upregulate things that we lose with time, thus restoring youth to the treated area.”

Are biostimulators a good alternative to filler?

If smoothing away fine lines and wrinkles as well as improving overall skin quality is your aim, biostimulators are a really good option, especially as they don’t overly plump your face.

“Younger and older patients alike are weary of looking overdone,” says Professor Khanna. “Fillers can be over used, with patients ending up with aesthetic downgrades via inappropriate treatment.” In contrast, “innovative developments in the biostimulator industry are making them more effective than ever before.”

Is there anything biostimulators can’t do?

However, as mentioned, “biostimulators are not true volumisers, and don’t always target the precise area that filler does,” says Dr Soni. “If patients are used to having nasolabial and marionette line filler, or jawline sculpting, for example, you can’t recreate those results simply with biostimulators.”

What’s next for biostimulators?

Still, the future looks very bright indeed for these injectable skin boosters – with aesthetic doctors using them both to maximise the results from other injectables – and, even perhaps, helping you push back a mini facelift…

“The forward-thinking clinician ought to look at combining fillers and biostimulators for their patients so that they get the best of both types of products in one session,” says Professor Khanna. He regularly teaches such combination techniques to clinicians all over the world.

“We can also delay the need for more invasive plastic surgeries with the introduction of [biostimulating] products  – and this is coming from a plastic surgeon!,” says Dr Soni. “The ability to provide a lift to facial tissues, reduce sagging and jowls, and improve the quality of the skin, is pretty amazing to get from an injectable product. I feel as though we can really help patients who previously would have jumped straight for a face and neck lift.”
