Hi Alice, really loving your website and content. It’s so helpful to learn a bit more about this side of beauty and the pros and cons of various procedures. I’m yet to explore this for myself or have anything done, but I know that when I do I will be so much more informed! My question is whether at home microneedling is effective on the decolletage area? I am in my early 30s but have creases in this area from being a side sleeper. These started in my 20s and I think there’s also a genetic factor at play, but I would like to do something about it as I am conscious of them. Thank you so much.

Home microneedling creates tiny punctures in the skin, so any products you use afterwards will sink in more deeply, and if this is something like skin-strengthening peptides or a retinoid, they will hit home rather harder. So it may help. My issue with home needling is that it is very easy to get overenthusiastic and overdo it; to do it patchily; to scratch up and damage the surface of the skin, and you need to be really scrupulous about cleaning the needling device afterwards (to prevent infection), and changing the head regularly. Also, home needling doesn’t go deep enough to stimulate collagen production in the skin – for that you need clinical microneedling, and yes that absolutely works on the decolletage as well as anywhere else on the skin. One last thing – don’t forget daily sunscreen on your neck and decolletage. It really helps preserve your skin for the future.

Have you reviewed the Lyma pro Laser

Hi, no I haven’t reviewed the Lyma Pro – the PR asked me if I was interested in it, and I said yes, only to be told they suddenly had no samples available.

Here’s what I’ve said before about the Lyma before:

Yes I have tried the Lyma ‘laser’ but I’m not a fan – I used it every night for months on end in spring 2021 (on one side of one wrinkle on my neck, and on the knuckles of my left hand, as advised by the brand founder,) without getting any results.
My laser-manufacturer acquaintances laugh at it when I ask them what they think of its supposed powers of rejuvenation. Why?
‘Look, this isn’t competition to us,’ said one. ‘Laser?’ laughed another. ‘That’s more of a laser pointer.’
The Lyma makes enormous claims, claiming the sort of results that it normally takes several rounds of in-clinic treatment to achieve, and has the flimsiest scientific backup  – and despite making these claims the company hasn’t seen fit to put the product through a single clinical trial or comparison with other devices.
It has been brilliantly and stylishly marketed and it has persuaded a lot of journalists who should have asked a few more questions to repeat its extravagant marketing claims. What should they have asked? How can a device the size of a torch with a rechargeable battery generate enough power to do what it says it does? Or how can any light which is allegedly strong enough to prompt healing deep within the skin be safe to use around the eyes/ shine directly in your eyes without protection?  Why do they suggest using an oil or cream with the product to give it ‘slip’, when any oil or cream will distort the light beam that’s reaching the skin?
For what it’s worth, if you are contemplating spending that much money, I know the Dermalux Flex works a treat because I gave it full road testing during lockdown #1 and got brilliant results. Plus it has a medical CE certification which means it can make medical claims to treat, for example, acne and heal wounds. That’s why I have added the Dermalux to the Tweakments Guide shop, and haven’t added the Lyma. I know which one actually works.

62 year old with generally good skin but a little laxity. Have had Botox and skin boosters. For laxity around jowls would you recommend neogen or software?

Both NeoGen and Sofwave can be useful for tightening loose skin, I would recommend finding a great practitioner and asking them which of these devices would be more appropriate for your skin, take a look at the Find a Practitioner section to find a recommended practitioner in your area.

How do I know if I have rosacea?

The tricky thing with identifying rosacea is that it shows up in many different forms. Apart from making the face look flushed, rosacea can show up as tiny raised red bumps or acne-type pustules, or an increased number of thread veins on the face. Add to that the way it tends to come and go, and you can see why it is tricky to spot, and why it isn’t diagnosed as quickly or as often as it should be.

Rosacea can be very mild, where your skin just has the odd tendency to flush — or it can be very obvious and persistent. It tends not to be obvious before the age of 30, and it tends to get worse the older you get. If rosacea is left untreated, it can thicken the skin, particularly on the nose, a condition called rhinophyma.

One of the odd things about rosacea is that it can make the skin feel rough and dry, rather than oily — people tend to expect the latter when they see spots. Rosacea is widely under-diagnosed, not least because not all rosacea looks the same.

I was recently recommended Heliocare oral sunscreen tablets after having Morpheus8. I’d never heard of oral SPF. I’m a religious user of the stuff on my face, neck and hands and so was interested but what about the science. Have you come across it? https://heliocare.co.uk/products/oral-capsules

Hi yes these are brilliant and the science is solid, they enhance the skin’s defences against UV damage. They’re not an alternative to sunscreen, I would always recommend using sunscreen and so would Heliocare, but if, say, you have someone who simply won’t (or can’t) use sunscreen but is prepared to take a tablet, they’re really useful.

Do you have any recommendations for hair brushes and combs? I have quite brittle hair and would appreciate advise on products that don’t make it worse

Tweak of the Week: Ultherapy® Brow Lift at the Cosmetic Skin Clinic – what was the result did you get the brow lift, are you happy with the results?

There was a result, but it was minimal – I’d check with your practitioner whether you’re a good candidate before committing to a treatment like this.

due to weight loss laxity and crepe-iness under my chin an on my neck I intend to have a full surgical neck lift but am currently reloosing some gained weight first. I am finding the neck very hard to live with and am considering going for some sort of high energy technique in the interim but I seem to remember Alice talked somewhere about some surgeons disliking eg ultherapy before a neck lift because it ‘mashes the skin together’ etc. are their any lifting Tweakments that don’t do this? For example Alice seemed very pleased with her results with the Profound RF treatment with Dr Shotter – does this method cause similar issues though? how serious is the trade off – would it make the neck lift significantly less good?

Hi, you’re right, cosmetic surgeons dislike patients having energy-based treatments (eg ultrasound, or RF needling) in the year or two before surgery because the way the treatment contracts and tightens up the collagen makes it trickier for them to do their job when it comes to separating the layers of the face or neck. Check this with your surgeon, but I’d hold off and wait for the surgery. I hear that you are finding your neck hard to live with but if you are looking at a surgical neck lift, you will find RF needling doesn’t give you the result you are looking for.

Is it worth getting a laser home use device ie. Nira pro as well as the Dermalux led device which I have

If you’ve got a Dermalux, I’d stick with that. Home-use lasers tend not to be very high-powered or effective.

Due to have sofwave. My husband wants to know what could go wrong. No mention of this was made by Dr Shotter.

Ask Dr Shotter or her team to take you through any potential side effects, risks,  complications or contraindications, these will be listed on the consent form. Also, if it’s any reassurance for your husband, Dr Shotter’s staff are well trained in the procedures they offer, like Sofwave, so you will be in good hands.

I’m looking at the profound rf, sofwave and EndoLift I have seen you have had them all. Which one gave you better results for your jawline/ general lift? And how long was the recovery compared to ultraformer? Thanks

Hi, Profound RF (which I wrote about here) gave me the most definitive result of these – but it’s an intense procedure that left me bruised and swollen for the best part of a week.  I’ve haven’t tried ultraformer but most ultrasound-based treatments hardly need any downtime.

Would love to try NeoGen Plasma but I have an autoimmune condition. Could you tell me if it is safe?

Hi, that’s a question for the practitioner with whom you are thinking of having the treatment. It depends on your autoimmune condition and how you are doing with it, to work out whether any treatment is advisable.

Endolift by a top practitioner has left me gaunt and with rosacea as well as unexpected fat loss I’m devastated and need advice please. I feel the need now for a face lift. This was done March 2021 please can you advise me

Hi, I’m sorry to hear this – there’s not much advice I can give. I am sure you have already considered fillers for restoring volume to your face. If you decide to move ahead with a face lift, go and have consultations with at least two surgeons, and then take time to assess their advice before booking anything.
